When an adult at risk dies from or suffers from serious harm, a SAR is conducted to identify how local professionals and organisations can improve the way they work together. A Safeguarding Adults Board (SAB) would usually make the decision to instigate a SAR and a report will be produced to document the findings and recommendations. This is a count of individuals involved in SARs which concluded during the reporting year. Individuals involved refers to the adults at risk.
